Abstract

The overhaul activity of a ship's main engine is one of the maintenance activities with a high level of complexity because it involves interactions between humans, mechanical equipment, engine components, and the working environment. This complexity may lead to potential occupational accident risks and system failures that can affect personnel safety and the operational reliability of the vessel. This study aims to analyze occupational accident risks and potential system failures during the main engine overhaul activity of MV CTP Fortune and to identify the contributing factors and appropriate risk control strategies. This research employed a descriptive qualitative approach using a risk analysis method. Data were collected through field observations, interviews with eight informants involved in ship engine operations and maintenance, and documentation studies related to main engine maintenance activities. Data analysis was conducted using the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) approach to identify occupational accident risks and Failure Mode and Effect Analysis (FMEA) to analyze potential engine system failures. The results showed that main engine overhaul activities are closely related to human factors, equipment conditions, work methods, and environmental conditions, which may influence safety risks and system reliability. Risk identification indicates the importance of implementing control measures through appropriate work procedures, equipment inspections, effective communication, and risk-based maintenance strategies. This study concludes that the integration of occupational accident risk analysis and system failure analysis is essential to improve workplace safety and maintain the operational reliability of ship main engines.
